We are having a few orders set as queued when they should be processed. The customers are skipping right over the payment information section and not entering a credit card? Shouldn't this be required?

This is caused by a conflict with the Google Chrome browser. Enable this option to fix this:

General Settings -> Modules -> One Page Checkout -> Display payment options as radio buttons:

You will also want to make sure this option is enabled:

General Settings -> Payment options -> Use dynamic (JavaScript-based) Credit Card info validation:
